Watch out for Brownout Warning in your boot log.

If you're having grief with the module and can't get to the bottom of it, check the boot log for "Brownout Warning". This just bit me in the _uts.
My USB port power was overloaded and wasn't supplying high enough voltage to the module.

Very good point. Dodgey USB leads/connections is a very common issue that many blame on the ESP/Code.
